CE Attendance & Evaluation Survey
Thank you for attending a continuing education (CE) event provided by OPHA! Completing this form after participating in an OPHA CE event serves several purposes. 
  1. It allows OPHA to track CE events attended.
  2. It allows OPHA to collect and evaluate an individual's event engagement for the purposes of reporting  continuing education (CE) units/contact hours to respective credentialing agencies (NCHEC, NBPHE, Social Worker, etc.).
  3. It allows OPHA to provide you CE certificates and certificates of attendance and evaluation quarterly.
Please note:
  • All efforts are made to properly assign certificates of attendance and evaluation. When you complete this form in its entirety, we can then best document your participation.
  • Credentialing agencies themselves deem CE eligibility and issuance of CE's. We strive to meet their standards and competencies.
  • Attendance at an OPHA CE event does not guarantee CE credit. CE-seeking attendees must complete this survey so we may ensure attendance and demonstrated learning.
