This screening tool must be completed by all individuals attending a daily scheduled event in Landmark or neighbouring community arena.  Up to three individuals can be screened per entry: player (required) and parent(s)/guardian(s)/sibling(s).  This must be done prior to the start of the event and before entry into the arena.
