※他大学の科目を履修する場合にこのフォームを使って申し込んでください。Please use this form when you are applying for courses provided by universities other than your home university.
※大学によりICL科目の成績確定時期並びに成績証明書発行時期が異なるため、ICL科目により取得した単位が在籍大学の単位互換申請期限に間に合わない場合がありますので予めご了承ください。Please be noted that ICL course grades and transcripts may not be available in time for one's home university's credit transfer deadline due to differences in the timing of finalizing the grades among the ICL universities.
〔単位互換に関わる個人情報の取扱いについて〕[Handling of Personal Information for Credit Exchange]
本申請書に記載される個人情報及び申請者の成績評価についての個人情報は，ICLコンソーシアムにおいて単位互換協定を結ぶ各大学が，単位互換制度を運営するために必要な次の利用目的に使用します。なお，利用目的達成のために，各大学間で相互に必要な情報の提供を行います。①単位互換協定に基づく各大学間の所属大学と協定大学間の派遣・受入手続業務②特別聴講学生のための学籍管理業務（履修登録・成績評価・単位認定・学生証及び証明書発行等）③単位互換制度を維持管理するための業務
Personal information recorded by the application form and information on applicant grades will be used by universities participating in the credit exchange agreement for the below objectives in order to operate the credit exchange system. The participating universities will exchange information needed to fulfil these objectives. ①Procedures related to outbound/incoming students among the affiliated universities and partners, as per the credit exchange agreement ②Managing the enrollment of Special Auditing Students (course registration, grades, credit approval, student IDs, issuing certificates etc.) ③Tasks related to the ongoing administration of the credit exchange system
