Please fill out the below form to sign up for middle school water polo. Participants must be students of KENTWOOD PUBLIC SCHOOLS District.

Swimmers must complete and turn in physicals, pay-to-play, and the middle school athletic code to their school secretaries ASAP. Please check the team packet for required team gear and order form. Orders are due by 1/17/20

6th Graders do not have to submit pay-to-play but must turn in a completed KIDS form and payment to coach on the first day.
