If your college/university requires a mid-year (7th semester) transcript, regardless of how you applied, please fill out this form. Complete only one form for all colleges/universities.

Mid-year transcripts will be processed in late January after 1st semester grades are finalized.

If there have been any changes to your 2nd semester schedule, it is your responsibility to communicate this information to your colleges.
