Mandatory Travel and Health Declaration

To protect and safeguard the ARCH tribe, we are asking all visiting guests to complete the following declaration form before entering our studio or event space. Anyone (including airline crew and HK residents under the Return2hk Travel Scheme) who has returned or lives with anyone who has returned from overseas (including Macau and Mainland China) must refrain from visiting our studio/event space for 14 days commencing their date of return. There are no exceptions to this requirement.

COMPULSORY DECLARATION

If you have selected "Yes" to any of the questions below or are unable to complete the Declaration, you will not be allowed to enter our studio. If you have a fever or respiratory symptoms, we advise that you rest at home and/or seek immediate medical attention.
