Kittens should receive their first FVRCP vaccination at 6 to 8 weeks of age, followed by three booster shots given once a month until 11 weeks of age. Do you agree that you will continue to protect your kitten with these vaccinations with the recommendation from a veterinarian? *
You understand that you are applying to adopt a cat/kitten whose medical history is unknown. Cats are tested for FIV and FeLV (leukemia) before placement and we cannot anticipate future medical problems which may arise. You agree that, as the cat's legal owner, all future veterinary costs are your sole responsibility from the date the adoption contract is finalized. During any "foster-to-adopt" period, the cat(s) will still be under the care of the rescue. *
The average expense of a healthy cat is $3000 to $4000 per year, to include food, litter, environmental enrichment (toys) and yearly veterinary preventative care. Do you agree to make such a financial commitment to this cat? *

Over-the-counter flea and tick preventatives (Hartz, Adams, etc) can be extremely dangerous to cats and dogs. Reactions can cause problems such as skin inflammation, seizures and death. Do you agree to only use veterinarian-approved products such as Revolution Plus, Frontline, Advantage, Capstar and Bravecto? *
