USOLMT Board of Directors Application
Thank you for your interest in serving on the USOLMT Board of Directors.  Please fill out this form in its entirety.  Screening questions regarding your criminal history and history with bankruptcy are required to be answered honestly.  Each board member of USOLMT will be added to our Board Roster with the Arizona Corporation Commission.  Any person applying for or serving on the board of USOLMT, a Registered Non Profit Corporation, will be disqualified from holding office if they have (1) Filed Bankruptcy on any company or organization within the last 10 Years or (2) Have ever committed any crime including misdemeanors or felonies that include financial transactions, theft, check fraud, and similar crimes.  Applications are reviewed by the Board of Directors.  All answers will remain confidential.  You will need the names, email addresses, and phone numbers of 3 professional references to complete this application.
