Sign Up Form for ALL Campbellsport Enrolled Students for the Breakfast/Lunch Virtual Learning Program.

This form is to request Breakfast and Lunch pick up through the Virtual Learning Program for the 2020-2021 school year. Please indicate what meals you would like to pick up.

Starting the week of September 14th, 2020:

*Pick up will be once a week EVERY Tuesday at select Campbellsport School District locations BETWEEN 9-9:30 AM ONLY. Please contact Cheryl Hintz at (920) 533-1283 prior to pick up,  if you are unable to make the scheduled pick up times.

NOTE: You will need to submit weekly the prior Friday by 3 PM.

Reminder that students are not charged for meals via USDA/DPI Food & Nutrition updates to the SSO waiver for Campbellsport School District through May 28th, 2021.

NOTE KEY UPDATES FOR FALL: The meal program will be available only to children who are in grades K-12, not all children in the house hold AND they must be attending Campbellsport School District.

The district will provide meals for school days only. Meals will not be provided for weekends, holidays or breaks or other non-school days.
