ASSUMPTION OF RISK I acknowledge that participation in or attendance at any activity and/or event associated with the facility involves inherent risks and dangers of accidents, serious personal and bodily injury, including death, and property loss or damage. I have considered and evaluated the nature, scope, and extent of the risks involved, and I voluntarily and freely choose to assume these risks. RELEASE FROM LIABILITYI fully and forever release and discharge the Companies, and their respective parent companies, subsidiaries, directors, officers, employees, agents, insurers, assigns and successors, and all other involved in the activities and/or events including but not limited to coaches, managers, site coordinators, referees, advertisers, and, if applicable owners and lessors of premises used to conduct the activity and/or event any related events and/or activities (the Released Parties) from any and all losses, damages, injuries (including death), claims, demands, lawsuits, expenses (including attorney fees), and any other liability of any kind, of or to me or any other person, directly or indirectly arising out of or in connection with my participation in or attendance at all activities and/or events, including transportation related to the activities and/or events .COVENANT NOT TO SUEI will not initiate any lawsuit, court action or other legal proceeding against the Released Parties, nor join or assist in the prosecution of any claim for money damages which anyone may have, on account of loss, damage, or injury sustained by me or others in connection with my participation in or attendance at the activities and/or event, and I waive any right I may have to do so. This means that I cannot sue to hold the Released Parties responsible for any loss, damage, or injury that I may experience related to all activities and/or events.NO SUBROGATION I waive my insurer’s right to make a claim against all parties involved in the programming. the Released Parties based on insurance payments made to me or on my behalf for any reason. It is the policy and practice to maintain all information given to us as private. All information given to us in confidentiality is kept secure and inaccessible to any other companies and or persons.  will not assume responsibility for any injury sustained on the premises. The family of the child hereby waives all claims for injury, damage, or loss to his/her person and property during his/her participation. This also releases the trainer and staff from any liability for injury, damage or loss that may be caused by any act or omission of any of the person(s) while on the grounds of the training .Pictures and/or videos of the athlete named below may be taken to use for promotional purposes.
