Dear Parent or Guardian,

From time to time, we have days we like to reward our students for working so hard by showing a movie (the students vote on which one to watch). We also may show one after reading the book together in class. Oftentimes, movies based on books are PG-rated. We need your signed permission to show these films. Rather than send one form home for each movie, we are sending this home for the whole school year.  Some (not all) movie choices are listed below:

Stuart Little 2
The Lorax
Brave
Wonder
Tangled
Maleficent
Frozen 2

Please note that the movies listed above are not the only choices students will have.
