Please complete this form to purchase your Sew-U-Nique Masks. The following steps will help you purchase masks with ease.
1. Check out Sew-U-Nique's website to determine which mask(s) you would like to purchase.
2. After you have determined which masks you would like to purchase, place your order here using this form. You will need to know the quantity and name of the fabric for the mask for you would like.
3. Once you have completed the mask order form, send your payment via:

NOTE: If you would like for your order to be shipped, please add an additional $5 for order of 9 masks or less and $8 for orders of 10 masks or more.
4. Once you have sent your payment, an associate from Sew-U-Nique will contact you within 3 business days with information pertaining to your order.
