The event organizers need the following information in order to process your request. None of the information will be shared with other entities. Due to COVID 19 you will be asked a few questions twice. We thank you for your patience as we endeavor to provide you with a safe experience.
You will need to fill out this document for each person in your group who plans to attend. We request if you have underlying health condition you do not attend the event.

COVID 19 requirements for Non-CUNY guests. You will need to register your COVID 19 data in CLEAR4 to enter a CUNY building. Please follow the instructions below.

If you are a non-CUNY guest. Please self-register by copying and pasting the link at the bottom. The link is for City Tech. Select the Visitors (New Employees Who Do Not Yet Have CUNYfirst Access, Guests, Contractors, Vendors) tab, or use the QR code. https://www.citytech.cuny.edu/comingtocampus/

Please upload your vaccine information or negative test results. Detailed COVID requirements are inside the link. You will need to have a negative test within 10 days of the event if you are not vaccinated.
