2019 KIVA CLASSIC ENTRY FORM

NOTE #1: If you enter more than one doubles draw, you need to be at the Red Chile sponsor level or higher.

NOTE #2: All players need to be prepared to play Thursday at noon. Those playing in two doubles draws need to be prepared to play as early as 8 a.m. Thursday, and may need to play back-to-back matches.

DEADLINE: The last day to register for the Kiva Classic tournament is Saturday, Nov. 16, 2019. We will post draws on US Squash by Tuesday evening, Nov. 19.

CHANGES: If you need to revise your entry, please email Tournament Chairman Grant Davis (grantcdavis@me.com).

We will be sending you information about how to pay by credit card. (At the club, we'll be taking payments by credit card only -- no cash or checks.)
