School Name *
Number of times the Bible Lesson is taught each club day. *
If your club breaks up into more than one group due to size- how many groups are teaching the Bible lesson? If your club stays as one group the entire time, your answer should be  "1". If you break up into 1st-2nd, 3rd-4th and 5th-6th grades, your answer would be "3"
Number of Students Enrolled to Date *
This is NOT the number of registration forms you have... but how many children HAVE BEEN TO YOUR CLUB AT LEAST ONE TIME. This number will never get smaller.  
Professions of Faith *
This is NOT the number of children who responded to the invitation.  Some of those had to go to the bathroom, prayer requests, needed assurance, etc. How many children were counseled and made a profession of faith for the first time THIS MONTH? We will add up your total for the year. This is just for the month you are reporting.
Number of Kids Starting Sunday School or Church *
If you don't know, put "0". Please do not guess.  Only count kids you know have come to your church or shared with you or a volunteer that they started attending church. If you don't know, that is totally okay.
Did you teach a party club this month? *
This is NOT the lesson 6 review.  This is one of the special party clubs for Christmas, Easter, Thanksgiving, Ice Cream MESSage, etc.
