I herby give my consent to Simulation Councils Inc. dba: SCS and The Society for Modeling and Simulation International.

(a) To record my likeness and voice on a video, audio, photograph, digital, electronic or other medium and to use my name in connection with these recordings and

(b) To use, reproduce, exhibit or distribute these recordings in whole or in part in perpetuity in any and all media throughout SCS (including but not limited to print publications, website, video tapes, hosting platform, non-theatrical home video, CD-ROM, internet and any other electronic or other medium presently in existence or invented in the future) this also includes use with online meeting applications such as Zoom, WebEx, Skype and Slack.

(c) It is also understood that the use of these recordings; audio and video is necessary to fulfill the conference requirement of author participation for the SummerSim 2020 Virtual Conference, which the author is a registered attendee.

(d) Recordings will be produced only by SCS personal, SCS volunteers and SCS contracted vendors only to satisfy requirement (c) and will be hosted by SCS’s third party vendor for one year after the event and then removed.

I hereby release SCS from and against any and all claims, demands, actions, causes of actions, suits, costs, expenses, liabilities and damages whatsoever that may hereafter have for any violation of any personal or proprietary right we may have in connection with such use of video, voice or name in any medium, I understand and agree that all such recordings in whatever medium shall remain the property of SCS.
