This form is to be used by parents and/or guardians or students aged 18 or older to authorize Fairfax County Public Schools to release student records and related information to support student applications to colleges, universities, scholarships sponsors, employers or other similar organizations.  This form will be filled out separately for each transcript request.

The first three transcript requests are free of charge.  Each additional request is $5.  Transcript requests submitted after LBSS transcript request deadlines (see below) will be charged an additional late fee per transcript.  LBSS cannot guarantee on-time delivery for requests submitted late.

Please review the following items before submitting this form:

1) Please complete the IS-111 Consent for Release of Student Records in Support of Postsecondary Application form so that your transcripts can be sent.  https://www.fcps.edu/sites/default/files/media/forms/is111.pdf  and email to LBSSTranscripts@fcps.edu  **Transcripts and other student records to support the student's application process will not be sent unless this form has been submitted.**

2) Submit the LBSS Counseling Self-Evaluation Form and the LBSS Parent/Guardian Questionnaire to the senior's counselor.

3) Payment:  The first three transcript sent to colleges are free of charge.  Additional transcripts are $5 per request.  Transcript fees can be paid using the student's My School Bucks account.  Go to myschoolsbucks.com and search "Transcript Requests" in the School Store.  *Transcripts will not be released until the fee is paid.*  

4)  The transcript package includes a current transcript, a Secondary School Report, a Counselor's Recommendation Letter if requested, and a LBSS School Profile.

5) Prospective athletes registered with the NCAA must list NCAA.  

6)  Lake Braddock Secondary School does not send test scores to colleges.  You must request test scores to be sent to colleges via College Board (https://www.collegeboard.org) or ACT (www.act.org)
