Entry form - Independent Teams
The 2022 National Day Cup on 1st & 2nd October is organized by the Hong Kong Lawn Bowls Association and subvented by LCSD.

Any vacancies left in the pool after the club entries will be made available to affiliated club members who would like to mix with other club players and form their own teams. It will be on a first come first serve basis and would only be confirmed after entry fees payment is received.

Interested parties should complete the application form below. Entry deadline is 12th September 2022.

For the entry fees of $1000 per team, please make cheque payable to the “Hong Kong Lawn Bowls Association”. Payments must be received by the HKLBA no later than 12th September 2022.

All teams must follow the HKLBA dress code mentioned in bye-law 1.17.

Team names will be assigned by the HKLBA.

Declaration:

By entering in the 2022 National Day Cup competition, all participants declared that they are healthy, physically fit, and suitable to participate in the above activity. The Hong Kong Lawn Bowls Association and the Leisure and Cultural Services Department shall not be liable for any injury or death which any of the participants may suffer in this activity, if the cause of injury or death is due to their negligence or inadequacy in health and fitness.
