Min/Max: 6 swimmers/20 swimmers per class

Members (Will be charged to the account)
$90 (5 classes M-F)
Non-members (Payment information must be provided prior to first class)
$115 + $15 Facility Fee (5 classes M-F)
Level 5 - Swim Team Prep Clinic
Prerequisites:
Must be able to swim 100 yards of the pool unassisted
Swim 25 yards freestyle
Swim 25 yards backstroke
Have a working knowledge of breaststroke & butterfly
Junior Swim Program Registration Instructions/Guidelines:
1. Complete all information on the form below.
2. Availability is based on time and date registration is received.
3. Non-members must enter and exit the club in a timely manner (approx. 30 minutes). Non-members are allowed to attend their lessons and are not allowed to use any other part of the facility.
4. Pro-rating will be accepted for medical reasons only (doctor’s note required).
5. Parent/Guardians please sign the Financial Responsibility Agreement below.
6. No refunds after start date.
7. No refunds/make-ups for non-attendance or lack of student participation.
8. Swimmer placement is subject to swim instructor approval.
9. All non-members must put a credit card on file or pay with check prior to enrolling in classes.
