Once you have received and filled in the DBS form you have been sent via post please complete this form so that we can send one of our volunteer checkers to collect it and check your appropriate ID.

They will do this on your doorstep maintaining social distancing.

The documents they require to see must follow these guidelines:
The applicant must be able to show:

ONE document from Group 1.
TWO further documents from either Group 1, or Group 2a or 2b, below
At least one of the documents must show the applicant’s current address.
