MSD 2021 8th Grade Promotion Ceremony
You must complete this registration form and waiver in order to attend your student's 8th grade promotion ceremony in person. Complete this form only once per student – please coordinate with families members to ensure that only one form is completed per student.  

The ticket for the promotion ceremony will be a single sheet of paper with the student's name and assigned seats and arrival time for all six guests in your group. Schools will coordinate distribution of tickets.
