The Wauwatosa School District will be partnering with School Perceptions to administer a Student Life Survey to all students in grades 4-12. The survey is designed to measure a student's engagement at school. It will provide the District with answers to important questions regarding support, barriers to learning and ways for the District to increase student involvement in and out of the classroom.

Please use this form ONLY if you wish to opt your child(ren) out of receiving the Wauwatosa School District's 2022 annual survey. Any student in grade 4-12 who has not been opted out will receive a link to the survey. The survey is completely optional and all data will be shared anonymously with District leadership. Participation and/or responses will have no bearing on any student's grade(s).

If you wish to opt your child(ren) out of participating in the survey, you must complete and submit this form no later than 5 pm on Monday, May 9, 2022. Please complete one survey per child that you wish to opt-out.
