This form is the first step in nominating your child for testing to determine possible eligibility for the elementary gifted and talented program.  Please understand that the tests, surveys, and rating scales may NOT indicate placement in the Gifted & Talented Program. The Placement Committee will evaluate all evident collected from parents, teachers, and students and determine if placement into the program is needed.  Completing this parent survey will help in determining the educational needs of your child.
