We need this for the organization of accommodation.

Commitment to pay & go *

Participation in the course will require from you two things. First, you will need to be willing to pay the financial contribution, partially covering the travel and lodging costs, as specified in the course syllabus. Second, it will require you to be willing to walk around two kilometers in a mountainous terrain (on a path) to the Patejdlova bouda, the place where the weekend event will take place (see http://www.cuni.cz/UK-5197.html). Please indicate whether you are willing to commit to these two things.

Accommodation - mixed (co-ed) dormitories (rooms) *

The organization of the programme requires that teams have a room available for team work and in-team discussions. Given the space constraints of the accommodation facility, the best solution is to allocate the rooms by teams. This requires mixed (co-ed) dormitories. In general, most rooms are shared by 3-5 people. Please indicate whether or not this is acceptable for you. If not, we will seek to accommodate also the preference for single sex rooms.

Dietary requirements

Please indicate if you have any dietary requirements. The facility does not provide any special diet, but please let us know anyway so that we can look into possible solutions.
