中国渡航歴のある学生（コロナウイルスに関連して）は以下の質問にご回答ください。
In preventative act JFOU requests all students to fill out the form if you traveled to China recently.

発熱（37.5℃以上）や呼吸器症状がある場合は、大学に登校する前に近隣の医療機関に連絡をとり、その指示に従って対応をしてください。診断結果を大学に提出してください。
