I do hereby covenant and agree to release, indemnify and hold harmless the Town of Pawling, all of the Town’s elected and appointed officers, employees, volunteers, and/or agents, from and against any and all liability, loss, damages, claims, or actions (including costs and attorney fees) for bodily injury and/or property damage, to the fullest extent permissible by law, arising out of the participation in Pawling Recreation Programs/Events/ActivitiesI understand participation in Pawling Recreation Program/Events/Activities involves rigorous physical activity and risks of physical injury, and we assume these risks. I hereby give consent for emergency transportation and treatment in the event of illness or injury. I hereby accept responsibility for the payment of any emergency transportation or treatment on behalf of the participant. I further certify the participant is in good physical condition, and has no medical or physical conditions that would restrict his/her participation in this event. I understand there are risks of physical injury in participating in sports and recreational activities or programs. I hereby release the town/city of Pawling, its employees, officials and agents from any and all liability or loss or damage to personal property that, my child or I may experience in connection with activities sponsored by Pawling Recreation. I hereby consent to emergency medical procedures deemed advisable for my child in the event I cannot be reached and my child has sustained an injury. The Dept. does not provide accident or hospitalization insurance for participants of its programs. All participants are advised to have adequate personal coverage. Please consider participant's own health, experience, and tolerance for risk before participating in any program. I also consent to the use of my or my child’s photo, video, artwork etc. by the dept for flyers, presentations. *
