Application for Denton ISD Instrument Usage (Band/Orchestra)
Denton ISD provides high quality instruments to its band and orchestra programs. The instruments provided by the district are typically the larger, more expensive instruments that are not available for rental by instrument vendors. The Denton ISD inventory also contains some of the smaller instruments that are typically rented so that lower income students may have the option to pay DISD a lower cost instrument usage fee in order to have access to an instrument as a band/orchestra student. Students must qualify by income for fee reduction. Please see your campus band/orchestra director if you believe you may qualify or if you have any questions. (For 2021-22, all DISD students will receive lunch for free. However, the income qualifications for fee reductions will still be applicable for the Instrument Usage Fee.)

1.  This application form must be completed and submitted by the parent/guardian of each student who is assigned a Denton ISD-owned band or orchestra instrument.

2.  Students using a district-owned instrument must pay the Denton ISD Instrument Usage Fee. This fee covers routine maintenance and normal "wear and tear" due to appropriate student use of the instrument. This fee does not cover damages due to student negligence. Repairs needed due to student negligence, misuse, or mistreatment will be charged to the student/parent. If the student loses the instrument assigned to him/her, the parent/guardian on this form will be responsible for replacing the instrument with the same make, model, and features.

Instrument Usage Fee Structure:
$125 - Woodwind, Brass, Percussion, & Strings (Grades 5-12, except 6th Grade Percussion)
$50 - Rate for students that qualify by income for Reduced Lunch
$25 - Rate for students that qualify by income for Free Lunch
$50 - 6th Grade Percussion

The Instrument Usage Fee may be paid online via InTouch or at the campus band/orchestra office via check made payable to "Denton ISD Fine Arts". The fee is due at the beginning of the school year when the instrument is assigned to the student. If a payment plan is needed, please see your band/orchestra director.

"I am requesting that my child be considered for a school-owned instrument to use while a member of the Denton ISD band or orchestra program. If approved to use a Denton ISD instrument, I accept full financial responsibility for the care of, damage to, and/or loss of the district-owned instrument. I further agree that I will return the instrument and any additional equipment issued by the district in proper condition upon request."
