Dance In Bloom - Birthday Party Waiver

A parent or guardian must remain on-site for participants ages 7 and younger. Participants who are "dropped off" (ages 8+ only) must have a parent/guardian reachable via mobile phone. They are to be picked up promptly at the scheduled conclusion of the event.
