Lean Six Sigma Program: No-Interest Payment Plan Request
Thank you for your interest in a No-Interest Payment plan to help manage the cost of attending a Lean Six Sigma training program. Please complete this form to initiate your payment plan application. A member of UC San Diego Extended Studies staff will contact you to finalize your payment plan shortly after submitting this form.

The programs tuition will be divided into equal payments, to be made on the 15th of each month prior to the classes completion. Tuition is typically broken into 4-6 equal payments, however, tuition can be broken up into as many as 12 payments, if payments begin up to one year in advance to the courses completion.

I agree to pay the total balance as outlined on the payment plan, which will be issued via email. I understand, if two payments on the scheduled dates fail, I will be dropped from the course without a refund and this agreement becomes null and void; “Holds” will be retained on my account, and no further enrollments will be processed until the debt is paid in full. In addition, I will not be eligible for any future payment agreements. Additionally, I understand that my card will be charged on the 15th day of each month and that there will be a non-refundable 2.75% service charge for every credit card transaction or a non-refundable $0.50 flat fee for every ACH (electronic fund transfer from bank routing/account) transaction
