Please follow the directions to complete this form. A description of each elective course is provided at the link below. A parent email is also required to complete this form.

1. Students: please include your parents/guardian's email for the first question. If your parent does not have an email, please use your email.

2. Students: Please answer each question. You must choose a different elective for each question. Most students are placed in their 1st or 2nd choice elective. Please answer each question carefully! Please note: some electives subject to change based on staffing/school budget. For music classes, advanced level will be offered provided there is enough student interest (if not, beginning and advanced levels will be combined into one class).

3. Teacher approval is required to retake an elective course. If you sign up for an elective you have already taken, the counselors will check with the teacher for approval.

4. Students: once you have made your elective choices, please ask your parent/guardian to review your choices.  A confirmation email will be sent to the email provided in the form. Please save this email for your records.
