(1) Employer Interest Application (this form)
(2) Signed LAEP Employer Agreement

(3) Posted detailed Job Description(s) for each student position on Handshake

(4) W9 / Certificate of Insurance (Optional)

After submitting this form, the SJSU LAEP Coordinator will contact you to provide important eligibility information and next steps.
