GUIDELINES:

The nominee must be actively engaged in community, civic, humanitarian, cultural, educational, Chamber, youth, and/or professional activities that have had a significant impact on the community or a segment of the community.
The nominee must demonstrate outstanding civic, business, and professional leadership, inspiring and positively influencing others.
The nominee should possess admirable qualities such as integrity, honesty, and a strong commitment to the betterment of the community.
Nominees can be anyone actively involved in Porterville and/or the greater Porterville area.
Current members of the Chamber’s Board of Directors are ineligible for nomination.

*Please attach your responses to the sections below. Be sure to complete the following with as much information as possible. Selection of the Man and Woman of the Year award is made from the information provided. No other research will be done by the committee.

**Nominations must be submitted to the Porterville Chamber of Commerce no later than 10:00pm on Saturday, August 31, 2024.
