FPBS Hotspot Request Form
If you do not have home internet access, please fill out the following Hotspot Request Form to confirm your need for an internet hotspot. If you have multiple children, we are only providing one hotspot per household without internet access, so you only need to fill out the form one time.

After completing the following information, your child will be contacted to pick up the device/charging cord from the LMC. All devices and charging cords must be returned back to Bayside Middle School or Stormonth Elementary by the end of the academic year.

I am requesting home access to a Kajeet mobile hotspot for internet access only to be used for academics by my child. *
I understand that my child will be issued a mobile hotspot for educational home use and my child is responsible for proper care, handling, security, storage and use of the device. I have read and discussed with my child, and agree to comply with terms and expectations in the District Handbook and accompanying District Board Policies.  I understand that my child's failure to follow the information and expectations outlined in this document may result in loss of device access, disciplinary action, and/or financial consequences. I understand that the devices and accessories issued must be returned by the end of the academic year.
