By submitting this form, I confirm I am a member in good standing of the Irving Lake Association. I am applying for permission to use the Irving Lake Association’s Cypress Cove Park facilities for personal use. As applicant I understand and agree to the following conditions of usage:

- Applicant will determine the suitability of the facility before submitting this application.
- At the conclusion of all reservations, the park site shall be left in a clean and orderly condition.
- Applicant agrees to pay the cost of repairing any damage to park property or equipment incurred by the group using the facility.
- All applicant’s possessions, supplies and equipment will be removed from the facility at the end of the reservation period.
- The group using the facility must vacate the facility at the end of the reservation period.
- Applicant shall be responsible for the conduct of all participants.
- Proper safety measures will be the responsibility of the applicant and must be exercised to protect the participants and park property.
- The Irving Lake Association will not be held liable for any loss or damage suffered by person or property connected to the exercise of this privilege. The applicant and group using the facility further agree to save and hold the Irving Lake Association harmless from any and all claims of every character whatsoever that may be made against it by reason of the operation of this activity.
- Applicant agrees to pay a clean up deposit of $50, which will be refunded once all of the above conditions have been met.

If member wants to use the porta potty, they can either clean and provide toilet tissue or pay to have the porta potty cleaned by a professional for $100.00
