Please fill out and submit this form to request full time remote learning for your child during the COVID 19 pandemic.  If you are requesting remote learning for more than one student you will need to fill out a separate form for each request. You will need the registration code provided to you to fill out this form. If you have lost the code please call the office at 217-469-2332

If you wish to have your child follow the 50% in-person / 50% remote learning hybrid plan outlined by St. Joseph-Ogden High School you do NOT need to fill this form out.
