By selecting "I agree" in the next text box, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and/or I may be exposed to or infected by COVID-19 by attending In-Person Lessons, Flute Choir Rehearsals, and/or any event pertaining to the Myrtle Beach Flute and Saxophone Intensive and the faculty on staff that such exposure or infection may result in personal illness. I understand that the risk of becoming exposed to or infected by COVID-19 at The Myrtle Beach Flute and Saxophone Intensive may result from the actions, omission, or negligence of myself and others, including, but not limited to, the faculty of the Intensive, volunteers, and program participants and their families.
I voluntarily agree to assume all the foregoing risks and accept sole responsibility of any ilness to my child(ren) or myself. On my behalf, and on behalf of my child(ren) or self, I hereby release, covenant not to sue, discharge, and hold responsible The Myrtle Beach Flute and Saxophone Intensive, its faculty members, volunteers, and representatives, of and from the claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating to. I understand and agree that this release includes any Claims bases on the actions, omissions, or negligence of the Myrtle Beach Flute and Saxophone Intensive, the faculty, volunteers, and representatives, whether a COVID-19 infection occurs before, during, or after participation in the Myrtle Beach Flute and Saxophone Intensive.

The Myrtle Beach Flute and Saxophone Faculty, agree to encourage social distancing, have hand sanitizer available for all participants, wipe surfaces down constantly with disinfectant wipes, encourage washing hands when leaving and entering, and encourage to wearing masks when applicable.

I understand and acknowledge the risks associated with being a In-Person attendant to the Myrtle Beach Flute and Saxophone Intensive. I further understand and acknowledge that my child’s use of the facility at Coastal Carolina University Edwards Hall for the Myrtle Beach Flute and Saxophone Intensive is solely at my own risk and I assume full responsibility. I hereby further declare that my child/self remain at home if sick, cover their coughs and sneezes, and wear a mask when applicable.

I furthermore agree to pay the amount of $350 to the Intensive and waive my right to sue you for any injury or medical issue that may happen to my child during the week May 28th-29th, 2020.
I further state that I have carefully read the foregoing waiver and release of liability agreement, that I understand the contents thereof, and that I have executed this release voluntarily through my "I agree" virtual signed agreement box below.
