NU Food Service Request Form
North Union & North Kossuth School Districts will continue to provide meals for our students during the school closure. Delivery will be available on Monday, Wednesday, and Friday. Meals will include two breakfast and two lunch meals per student.

Based on need and results of this form, distribution locations will be determined with an attempt to deliver to all communities in the North Union & North Kossuth School Districts.

Completion of this form will provide meals starting as soon as we receive our waiver from the state through Friday, April 10th.
