This form is to report incidents of racial abuse against students and staff, particularly BIPOC students and staff, share information, submit complaints and provide confidential or anonymous feedback to the District regarding experiences on our campuses or within our District. This form is monitored during regular business hours, M-F, 7:30-5pm. This form is not monitored 24 hours a day. If you have an emergency and need immediate assistance, please call 911.

Once we receive your confidential report, we will start an investigation and contact you, if you have requested to be contacted and if you have provided your contact information in the form below. If you have chosen not to provide your identity or contact information, we will review the information you submitted and determine the available next steps in order to address the matter.
Your submission will be stored in a password protected system only accessible by the District employees listed above.
