Hey Partner! - Before filling this form out, please make sure you are double checking all of the troubleshooting steps and making sure your credentials actually need a reset. - Please use this tutorial to guide and troubleshoot the log in process. Keep in mind, the Dynata system usually takes three to four failed log in attempts before a successful log in. (If the system does not let you log in right away, please try rebooting, and typing in your credentials a couple of times)
