There is no wrong way to fill this out, simply provide as much information as you feel comfortable providing.

Once you have filled out and submitted your form, please email a photo to lm@lesliemurphyarts.com with the subject 'memorial portrait'. Try to look for the following when selecting a photograph to be used for the portrait:
1. All facial features in focus
2. Directional light source (light coming from one side or the other)
3. At least 1mb in size and a .jpg file
