Thank you for signing your child up for Kids in the Woods! We are excited to get started.

If you are registering more than one child, please fill out one form for each.

The following form includes Registration Confirmation and Emergency Information. Please fill out this forms fully at least 7 days before the first day of camp. On the first day of camp, please be prepared to sign our Code of Conduct form and have the option to sign our Photo Release Form. Copies of both can be found at childrensdiscoverymuseum.org/camp.

All camps must be paid in full at least 7 days before the first day of camp. Without payment and this form, we are unable to hold your child's spot. This year, we are excited to provide online-payment, which you can access through the Kids in the Woods webpage. Visit childrensdiscoverymuseum.org/camp. You may also pay by sending us a check to Children's Discovery Museum, 7 Eustis Parkway, Waterville, ME 04901.

We have a very limited number of spots available and camp is limited to one-week per child.

All of the camp day will take place outside! In order to keep children safe in the sun, please send them to camp with sunscreen already applied. We encourage children to bring sunscreen with them as well to reapply throughout the day. *

I understand that it is my responsibility to send my child to camp with sunscreen already applied.

I, the parent/legal guardians of the above named child, hereby give my approval to his/her participation in this program. I hereby release, indefinitely and hold harmless the Children’s Discovery Museum, Friends of Quarry Road, the City of Waterville, its staff, its volunteers, and/or members of its Board of Directors from any responsibility, claims or liability in connection with my child’s participation in this program. I assume the risk of any accident, injury, or illness which may result from or during the program. In case of the need for emergency medical treatment, I hereby give permission for such treatment to be given if I cannot be reached.
