ASAA Volleyball/Basketball Zone Medal Return Form
15 medals of each colour were issued to you as zone host.  Unused medals (from this year's allotment only) may be returned and you will not be invoiced for them.  This form must be completed within 3 school days of your zone competition.  Zone awards invoices will be sent out on the 4th day.  Each host will bear responsibility and expense to return the medals to Catholic Central West Campus, Lethbridge c/o Shawn LeBleu.  I ask that all non-used medals are returned ASAP.  You will receive a separate invoice for medals not returned.  Medals currently cost $5 per medal.
