DUE DATE: PLEASE SUBMIT THIS FORM ON OR BEFORE JUNE 1, 2020.

Millikan students who will be in 7th and 8th grade in the 2020-2021 school year must complete the following questions which will help your counselors create a schedule that best meets your needs. All middle schoolers will be scheduled to take 1 Language Arts class, 1 Social Studies class, 1 math class, 1 Integrated Science class, 1 Elective, 1 PE Class and Homeroom.
Important points to consider during this elective process are: (1.) The choice you make does not guarantee a placement in the class. (2.) Many factors that go into each student’s elective placement. These factors include, but are not limited to grades, citizenship, teacher recommendation (i.e. Specialty Electives), space in the class, enrollment to run an elective, and staffing. (3.) You may be placed into an alternative elective class (a 2nd or 3rd choice) and/or intervention class, which will be decided by the administration based on the above factors; even if you complete this form. (4.) The survey window will close on Friday, May 29, 2020. Students who miss the survey window may not have their elective choices honored (depended on enrollment).
