Northwest High School Mid-Year Transcript/Report Request
Some colleges ask students to send Mid-Year transcripts/Mid-Year School Reports as part of their application packet. Please use this form to request mid-year transcript/school report(s).

Mid-year transcripts will be available to be sent from our registrar's office around the middle of February.  

Do not use this form for INITIAL transcript requests. If you have an initial transcript request, please follow the instructions on the Northwest website https://www2.montgomeryschoolsmd.org/schools/northwesths/counseling/#registrar
