1. I understand that as part of the application process, in conjunction with my position as a mentor for NMYCA, information may be collected that is of confidential nature. This includes information on both the Mentor and the Cadet. During the Residential Phase and the Post-Residential Phase, more confidential information may be obtained that is necessary to assist the cadets in the attainment of their future goals. All staff are prohibited from sharing privileged information with anyone without permission. Mentors are considered “UNPAID STAFF” and therefore fall under the NMYCA confidentiality agreement.

2. I fully understand that the information I may become privileged to may be of sensitive and confidential nature. It is my responsibility to abide by the confidentiality agreement for Mentors and Cadets.

3. I understand that all information collected is prohibited from being shared with any outside person, organization, or agency without the prior written approval of the RPM Supervisor, Mentor Coordinator, or Program Director.

4. Approval for release of information must be in writing, with justification for the release, from those listed in paragraph 3 above.
