This form is required to request tree removal (alive or dead) for homeowner or common property. Scan or send a picture of your property survey with tree(s) marked. Email to: Randall@agro-shield.com. Or, you can mail the survey to 69 Hardwood Rd., Glenwood, NY 14069.

After submittal, a member of the Woodlands & Habitat Committee will meet with you to guide and discuss remediation options.

PLEASE NOTE THAT A FINE OF $250 PER TREE WILL BE IMPOSED ON ANY COMMUNITY MEMBER THAT REMOVES A TREE WITHOUT PRIOR APPROVAL.
