This form is for those who are interested in using one of the EMS Bus Routes. If you think you might use the Bus, please enter your information below. This is *only* a registration form, not a commitment to pay. The business office will work up a total fee for your family based on the information you submit. Please contact Mike Stoltzfus if you have financial questions regarding EMS bus transportation.

Carefully review the following route and rate information and complete this online form by AUGUST 11TH. A copy of your submitted form, including the rate information below, will be emailed to you. You will also receive a link to return to the form should you need to make any changes.  

Please note: Rates are charged PER QUARTER. Rates are charged regardless of whether your student rides every day. A family rate is offered at an additional 50% of base rate (i.e. first student rider is charged full rate then each additional rider is charged 50% of subscription rate). All billing will appear on your FACTS statement and billing notifications.

EMS welcomes exploring alternative bus routes to serve you better. If you wish to make suggestions to improve the bus stop locations for your family, please email Mike Stoltzfus at stoltzfusm@emhs.net or make a suggestion in this registration form. Remember, an EMS bus pick-up and drop-off spot must be a safe "public" location; we cannot stop along roadways.

*Students wishing to ride the bus on a periodic basis must notify the business office (stoltzfusm@emhs.net) at least 24 hours in advance. Incidental one-time rates vary depending on route.

**Rates are based on the number of students riding the shuttle and the distance it has to travel to pick up or drop off your student(s). The following routes and stops are based on previous years' demand and usage. Please be advised these rates are subject to change.
