Super Summer Slam Day Camp will be June 28th through July 2nd, from 5:30 to 8:30 PM.

Parents, please fill-out one application FOR EACH child you wish to attend Super Summer Slam.
(After filling out for one child, you will have the option to submit another application.)

This is an Application. After we receive all applications, we will give you a notice after Memorial Day if your child has been accepted for camp this year. We will do our best to include all children from a family so no child in a family is left out.

Open for children who are AGE 6 through having completed Grade 6.

You will receive an email with your application answers.

Please double-check your information before submitting the form.

ESPECIALLY CHECK YOUR EMAIL ADDRESS. If it is incorrect we cannot contact you, nor send the required paperwork for your child to attend Super Summer Slam.
