Employee Health Self-Screening & Checklist
Employees MUST self-screen themselves for COVID-19 symptoms DAILY before they come to work and should they have any of the below symptoms they should not come to work that day.  Employees will be required to submit the signed certification below DAILY to their supervisor to show that you have been conducting daily screenings and are not coming to work with these symptoms.

Have you recently begun experiencing any of the following in a way that is not normal for you:  feeling feverish or a measured temperature greater than or equal to 100.0 degrees Fahrenheit, loss of taste or smell, cough, difficulty breathing, shortness of breath, fatigue, headache, chills, sore throat, congestion or runny nose, shaking or exaggerated shivering, significant muscle pain or ache, diarrhea, nausea or vomiting? *
Have you have recently had any contact with anyone that is/has tested positive for COVID-19? *
Are you currently waiting on a COVID-19 test result that you have taken as a result of doctor’s orders or recent exposure? *
By coming to work, you are attesting that you confidently answered “NO” to ALL of the above questions and have a very low risk of carrying or spreading COVID-19. If you answered “YES” to ANY of the above questions, please contact your direct supervisor immediately and email Human Resources at employeecovid@leanderisd.org
