I agree to the following:

I am aware that yoga is a physical activity and that I could injure a part of my body or trigger or exacerbate any medical condition that I may already have which may be affected by performing breathing exercises or from the performance of yoga postures or any other activity.

I freely accept and fully assume any and all physical risks to my body or my health that may result from participating in yoga classes or other activity, including the risks or health hazards and I accept all responsibilities of personal injury or loss that may result from these activities.

I accept that even with clear instruction, there is a possibility of injury and that it is my responsibility to consult a physician regarding my ability to participate before taking instruction with Nicole Catenazzi.

I agree to waive any and all claims that I have or may have in the future against Nicole Catenazzi, its teachers or any other person providing instruction in any capacity, and to release any and all liability for any personal injury or injury to my health or consequential loss I may suffer arising out of or as a result of my doing yoga including negligence on the part of Nicole Catenazzi, or its teachers or other employees or breach of any statutory or other duty of care.

I attest that I have no psychological, medical or emotional condition that would prevent me from safe participation in a yoga class or other related activity.

I recognize that this agreement of release and waiver of liability is a legal contract and that, by reading it carefully, I have complete knowledge of its contents.


Authorization of this Document will waive certain Legal Rights, including the right to sue. You acknowledge the sufficiency of the consideration exchanges for the Release.
